Office Policies


I understand that I am responsible for obtaining an insurance referral if my insurance carrier requires one.  If not, I do understand that I am responsible for payment.


Payment for treatment, deductibles and/or co-payments are due at the time of service unless other arrangements are made.
Sports & Family Chiropractic, Ltd.  will directly most bill insurance carries.  Please ask if this applies to you. Most insurance carries will not pay for supplements or equipment such as supports, tapings, pillows or acupuncture.  You will be expected to pay for these services or supplies at the time of service.  Please ask if you have questions regarding this policy.

Appointments – Lateness, Cancellations, No-Shows

In consideration of other patients, all patients are expected to arrive at the office at their pre-arranged appointment time.  Late patients will be attended to as time permits.  We ask that if you are going to be late, that you contact our office immediately.  Patients arriving more than 10 minutes late may be asked to reschedule.  After two (2) late appointments, you will need to reschedule and pay for your visit in full.  Accordingly, we try to deliver the same respect for your time.  If we are running late, your session will be completed in its entirety.

We reserve the right to charge your account for a missed appointment, or “no show.”  We will excuse one missed appointment in the event that you forgot or had an emergency.  However, any subsequent no-shows will be charged at the full office visit rate.  Three (3) no-shows may result in a patient being dropped from the practice.

If you need to cancel an appointment, please contact the office at least three (3) hours in advance.  We understand that emergencies and/or conflicts do arise, but we would appreciate notice as soon as possible.  Cancellations without three hours notice will be considered a “no-show.”
